Typically, an account will be suspended that quickly only if it's riddled with misspellings and grammar issues and shows a lack of proofreading. I've never included any titles or headers in my document. Your introduction and conclusion should clearly stand out on their own without being labeled as such. If you do include any headers, they should not be part of the wordcount as they are not part of the content.

It's best to only submit it to one category, otherwise if it's accepted multiple times then you may have multiple people trying to buy unique or full rights, which would lead to you violating the license. In general, client requests take precedence over general CC rules, as long as you're submitting it specifically for that request (and not into the general pool). I can't say for sure what happens if the client doesn't accept your request and it's released into the general pool.

I wouldn't worry too much about low sales recently. Sales tend to go in a cycle. You may have a few poor months followed by several months of selling almost everything.

I wouldn't consider it a run-on sentence because it's two halves of one thought. Essentially, it's an imperative in two parts joined by a conjunction. If your account was suspended, then all of your articles are no longer for sale. However, you do retain the rights to them and of are free to sell them elsewhere. You still need to abide by the terms of any sales that you did make (usage/unique/full rights). I have to ask what their rude message was...

We still retain the copyright when selling for unique rights. However, part of the sale is an agreement that we won't let anyone else license our copyright.
