Yes, title freezes after acceptance. However, the system will not save changes to Title after submission but BEFORE editor review. I think this is a bug.

if I edit the title field and click Save Changes, the changes are not actually saved. However, when an article is rejected, it is possible to edit this field, but only until the article is resubmitted, after which changes are no longer saved.

No idea - I assume it depends on workload, staff availability etc. I know that it sometimes takes over a week for submitted content to be edited.
