They use a "universal" login. I assume you have a different email/password combo for each account? Just sign in as the username/password for the writer account you just created. For example, I have a customer and an author account. I have a different user name for each.

That happens sometimes. I'd resubmit with a short note in the summary letting the editors know like this:
*** Note to Editors: 'Gouverneur' is Mr. Morris's first name. He is not a governor. *****

CC has two different ways to sign up: as a customer and as an author. Try logging out and then re-signing up as an author. You'll have to take a short quiz. Good luck!

Constant-Content may have a different take, but in my opinion it's probably best not to rely on others to adhere to terms of service you've previously agreed to and are on the hook for.

The requirement of the logo, a link back to your Examiner page (CC doesn't allow this), and "as published on Examiner.com" may be a problem. People that buy usage rights are not supposed to alter the text in any way, including removing a line such as "as published on Examiner.com"...

A couple of possibilities:
1 - One of the usage customers got ripped off by a content thief (which is beyond our control)
2 - One of the usage customers didn't understand the rights (which CC can possibly help with)

Either way, it does feel sucky.
